昨日の朝、また水っぽい分泌物が少しあった。前の日に母と電話で話して気になりだしていたので、朝からインターネットで情報収集。破水に気づかず、死産してしまった例なんて読んだら怖くなって、結局先週下腹部痛で行った分娩病棟に電話した。やはり、「生理パッドをして、また出たら電話しなさい」と言われたので、「分泌物だか、尿だか、羊水だか判断できませんのでチェックしてください」と強行にでて、Jasonと病院に行った。
まさしくも出産するためのベッドがひとつだけ置いてある小部屋に通され、お決まりの、助産婦による、検尿、血圧、赤ちゃんの心音チェックの他に、今回は検温もした。妊娠中期で早産となると、子宮か膣の細菌感染が考えられるからだろうか。助産婦に「生理パッドをちょっと見せてください」といわれたので、はずして見せる。「これは普通分泌物ですね」と、彼女。「今朝、水っぽいのが出たときはパッドしてませんでした」と、私。そして「この後、医者が検診しますのでお待ちください」と言われる。日本の情報だと、医者か助産婦がリトマス紙で「水」をチェックするだけなのに、医者が登場するの？と、オオゴトなのにちょっと面倒くさい。
ちなみに、今日の心音検査にかなり助産婦が戸惑っていた。普通は心音拡声器の聴診器をお腹にあてると、すぐに「ドッ、ドッ、ドッ」と聞こえてくるのになかなか聞こえない。かすかな音がしたりもするのだが、助産婦が何回もジェルを塗りなおしてトライする。「私は聞こえるけどあなたにも聞いてほしいのよ」というのだが、拡声器の乾電池の入ってるあたりがテープで補強されてて、それを押したりたたいたり、要するに器具がぼろいのだ。他の部屋からなにやら出産に使うらしいもっと大きな器械を持ってきたが、なぜかそれでも心音が拾えない。おかげでこっちは赤ちゃんの心音が弱いのかしらと心配してしまった。「出産が無事に済んだらこの病院に少し寄付でもしなきゃね」と、結構まじに小声でJasonに言う。
待ってる間に紅茶などを振舞われ、2時間半近くたってやっと、インド系（？）男性の医者と中国系の助産婦がやってきた。先週、下腹部痛で来たときに、出血、出水があったら連絡するように言われたこと、木曜日の朝と夜そして今朝また水っぽいのが出たことを説明すると、内診することになった。この医者、この病院あるいはこの国のやり方に慣れていないのか、助産婦に、器具の準備の仕方からなにまで教わりながらやっているのでちょっと不安になる。器具を挿入して、咳をするように言われる。子宮口は開いていなく、異常なし。細菌チェックのため、分泌物を少し採り終了。細菌感染していると水っぽい 分泌物がある場合があるらしい。来週の水曜日ぐらいに結果をGPに電話して聞くようにとのこと。
リトマス紙による「水」のチェックはなかったが、結果的には水っぽい分泌物だったらしい。破水だと、もう少しコンスタントにぽたぽた水が出てくるし、独特の匂いがあると助産婦に言われた。。「私には羊水と分泌物の区別がつかない」と、もう一度強調すると、とにかく、疑わしかったらいつでも戻ってきなさい、非常に重要なことだからと医者に言われ、帰された。
その後、カナリーウオーフのロイヤルチャイナに点心を食べに行った。今日は天気も気温も快適で、外で食べた。そういえば、カナリーウオーフに向かうバスの中で、初めて英人のお兄さんに席を譲られた。やっと妊婦ってわかってもらえるようになったのね！
「羊水、分泌物、尿の見分け方」について、今朝インターネットで調べた情報から追加。一度破水すると、動いたりするたびに少しずつ独特の匂いのさらさらした水が出る。妊娠中に膀胱が圧迫されて尿が漏れ易くなることがあるが、動いたぐらいでは出ず、咳やくしゃみで出たりし、尿の匂いがする。また、子宮頚管の分泌物が水っぽいことが妊娠中にある。それから、一度破水したらいつまでも気がつかないことはない。一度止まっても、また出るし、たとえ気がつかなくても、数日中に陣痛が始まるために異常には気づく。どちらにしろ、素人で判断は難しいので、疑わしい場合は医者に行くというのが基本。
I had some watery discharge again this morning. Since I talked to mom last night, I've started to worry again and I was browsing the Internet to get some more information. After I read a posting about a woman who had not realised breaking water and lost her baby, I felt more worried and decided to call the Labour Ward. As expected, I was told to call them again when I have some more again. I insisted, 'I can not tell the differences of baby water, urine and discharge.' Then Jason and I headed for the hospital.
We were put into a labour room which has a big bed with leg support, and a midwife checked my blood pressure, baby's heart beat, and urine sample. On top of that, my temperature was measured. Is it because bacterial infection usually causes premature labour in the second trimester? I showed the sanitary pad I was wearing. The midwife told me that it was usual discharge. I told her that I had not wearing a pad when I found some watery discharge this morning. We were told that a doctor would come to check me up. I thought my water was just going to be checked with litmus paper by midwife. Is it that big job? I was a bit surprised.
It was quite funny when a midwife checked baby's heart beat. She took so much time. Usually after putting the small device on my bump, you can immediately hear the sound. But we could not hear it. The midwife tried again for a couple of times. She said, 'I can hear it but I want you to hear the sound, too.' The device she was using looked dodgy. The battery lid was just stayed by tapes and a couple of button heads were missing. She pushed the battery lid hard and tried again and again. She then dragged in a much bigger device from another room, this must be used for delivery, and tried again but even it did not pick up my baby's heart beat. This made me feel a bit worried, is the baby's heart beat so quiet? 'We need to donate some money,' I whispered to Jason.
We ended up waiting for a doctor for about 2 and half hours. We were even offered some refreshments while waiting! An Indian (?) male doctor and a Chinese midwife finally came to see me. I explained that I admitted this ward for abdominal pains last week and was told to come back if I find bleeding or water breaking, I had some watery discharge on Thursday and this morning. The doctor decided to check the cervix. He seemed to be new to this hospital or even this country and the midwife instructed him how to prepare the device step by step. I felt a bit nervous. Is he ok? The cervix was not open. He took some swab for infection check, if there is an infection, we sometimes have watery discharge, the midwife said. I was asked to call the GP for the result at around next Wednesday.
They did not use litmus paper but I was confirmed that the water was discharge. The midwife told me that if baby water breaks, we experience more constant dripping with distinctive smell. I again told her that it is difficult for me to tell the difference between discharge and water or urine, she and the doctor told me to come back whenever I feel any doubt. These words made me quite relieved.
We went to Canary Wharf to have Dim Sum at Royal China after that. It was such a beautiful weather and we had a table outside. On the bus to Canary Wharf, I was offered a seat by a British man for the first time! I must look pregnant at last!!
Some additional information from the Internet about differences between baby water, discharge and urine. Once water breaks, a little bit or amount of water keep coming out when for example we move. During pregnancy, from the pressure of the uterine, the bladder muscle sometimes becomes a bit loose and we experience occasional urine leaking. But urine does not leak by only moving and it may leak when we sneeze or cough, and we can of course tell it from the smell. Discharge in the cervix sometimes become watery during pregnancy. In addition, once water breaks and even we do not realise it in the beginning, sooner or later we will know it as it keeps coming out and in a couple of days, contraction will start. In any case, it is difficult for us to judge and if we feel any doubt, better go to see a specialist.
