33W 3D 第三回出産前クラス　3rd Antenatal Class

第3回出産前クラスにRoyal London Hospitalへ。クラスの前に出産病棟ツアーに参加。分娩前、分娩、分娩後の3病棟を周る。

• 分娩前病棟(Early Pregnancy Unit): 私が妊娠発覚直後、出血騒動でお世話になった病棟。出産前に問題があって入院したり、予定日を過ぎて妊娠を促す処置が必要な時などに来る。
  
• 分娩病棟(Labour ward): 私が破水騒動で2回行った病棟。実際に分娩をする時病棟。水中出産用の部屋も見せてくれた。ただ、分娩ベッドが2台置いてある部屋は、ちょっといやだなあって思った。他の妊婦と同じ部屋でいきみたくない。混んでない限りは同室にはならないらしいが。
  
• 分娩後病棟(Postnatal ward): 出産後に入院。通常出産後6時間から48時間ステイ。帝王切開だと最低3日。希望により長期滞在可。パートナーは泊まれない。
  

さて、出産前クラスでは、主に出産後について以下の説明を受けた。

• 授乳
  
  赤ちゃんの吸う力は意外に強く、乳首がひりひりする。3週間ぐらいで慣れるが、諦めてしまう人も多いらしい。
  
• 会陰切開後
  
  水分を多く取り、尿を薄くする、水をかけながら用をたすなどの工夫で痛みを和らげる。ほぼ4週間で完治。
  
• ビタミンK予防投与
  
  新生児ビタミンK欠乏症予防のために、赤ちゃんにビタミンKが投与される。出産直後に注射するか、一ヶ月にわたり、3回に分けて飲ませるか選択できる。血液凝固に必要なビタミンKは胎盤から吸収しにくく、新生児の肝臓は未熟すぎて十分ビタミンKを生産しないことから、ビタミンKの不足がおき、新生児出血性疾患（胸、腹、皮膚、脳内からの出血）が起きる可能性があるため。また、粉ミルクにはビタミンKが添加されているが母乳には少ない。
  
• 新生児黄疸
  
  胎児は、血中のヘモグロビンを多く持ち酸素交換しているが、出産後に必要ない分は破壊される。この過程でできる物質が黄疸の原因。出産後2，3日で現れ、2週間ぐらいでひく。日光浴をさせ（光線が原因物質を破壊する）、授乳をきちんとする（原因物質を早く排出し、再吸収されるのを防ぐ）。まれに重症の場合あり。原因はさまざまで、助産婦訪問でもチェックされる。3時間間隔ほどの授乳に赤ちゃんが応じない時は助産婦に連絡すること。
  
• 助産婦の家庭訪問
  
  特に問題がない場合、退院後1，3，5，10日目に家庭訪問。その後はヘルスビジターが（定期的？）に訪問。
  
• へその緒
  
  おむつを替える毎にコットンに水を含ませたもので拭いてあげるとよい。出産後10日ぐらいで自然に剥がれる。
  
• 誕生5日目の赤ちゃんの血液テスト
  
  PKU(新陳代謝力チェック)など4種類の症状チェック。
  
• 赤ちゃんの体重チェック
  
  新生児は誕生10日後に約10％の体重減が普通。10日後に助産婦訪問で測る。
  
• 予防接種
  
  ２，３，４，１３月になにやらいろいろ接種する。
  
• レッドブック
  
  日本の母子手帳に当たるレッドブックは助産婦訪問の最後の日に取り上げられる。
  

Went to the 3rd Antenatal class at the Royal London Hospital. I joined pregnancy wards tour before the class.

• Early Pregnancy Unit: It's the unit I went when I had spotting just after I'd found my pregnancy. Only if you have some particular problems towards the end of pregnancy or you need an induction, you will go to this unit.
  
• Labour ward: Again, I went there twice when I had suspicious water leaking in about 20th week. This is the ward you actually deliver babies. We visited a room for water birth as well. I felt a bit intimidated when I saw a couple of rooms with 2 labour beds. I do not want to deliver my baby in the same room with another woman. It's only happening if the ward is really busy, though.
  
• Postnatal ward: We usually stay in hospital from 6 to 48 hours. (After the Caesarean, minimum stay is 3 days.) Can stay as long as you want. Your partner can not stay.
  

At the Antenatal class, we learnt:-

• Breastfeeding
  
  Babies suck breasts stronger than you imagine. You will get used to it in about 3 weeks. Lots of people give up before.
  
• After Episiotomy
  
  Taking plenty of water to dilute urine or peeing while pouring running water can help to reduce the pain. The cut will be recovered in about 4 weeks.
  
• Vitamin K
  
  To avoid Vitamin K Deficiency Bleeding, newly born babies are given either one-off Vitamin K injection or 3 dozes of Vitamin K by mouth over a month. Vitamin K is essential to clot blood but as it is difficult to be absorbed from the placenta, newly born babies are lack of Vitamin K, which may cause bleeding, in the worst case, from the brain. Breast milk do not contain much Vitamin K. We need to decide how our baby will be given Vitamin K or not at all.
  
• Jaundice
  
  Haemoglobin count of newly born babies is high as they used extra oxygen-carrying red blood cells to exchange O2 while still in the uterus. But once they are out, they do not need so much and during the breakdown process of the cells, there is a certain substance produced which causes Jaundice. About half the new borne babies develop Jaundice within a couple of days after birth but usually it's gone before 2 weeks old. Exposing babies under sunlight, which breaks down the substance, or feeding babies properly, which helps babies to get rid of or re-absorb the substance, will help. In any case, if babies might be jaundiced, doctors perform some test or photo therapy treatment.
  
• Cord
  
  Clean with cotton ball with water whenever you change nappies. In about 10 days, it usually comes off.
  
• Blood tests for 3 days old baby
  
  There are 4 tests such as PKU. It's done by midwife visiting home.
  
• Weigh baby at about 10 days old
  
  Normally, new born babies lose 10% of its original weight within 10 days from birth. Midwife visiting home weigh the baby at 10 days.
  
• Immunization
  
  New born babies are given immunisation shots at 2, 3, 4 and 13 months old.
  
• Home visits by midwife
  
  In general, a community midwife visit home on the 1st, 3rd, 5th and 10th days after backing home. After that, health visitors visit us (how often?)
  
• Maternity & labour red book
  
  It's taken away when midwife discharge us at about 10th day home visit.
